Admissions Information |
For Preschool Registration, You Must Bring 1. A state verified birth certificate. Children must have reached the appropriate age by August 15 in order to be registered. 2. The registration form completed with all information requested. Mark your 1st and 2nd preference beside the preschool class choice. Please print legibly and sign the back of the form. 3. A $10.00 non-refundable registration fee. Registration " Registration will be from 6 to 7:30 p.m. in the cafeteria and will be on a first-come, first-serve basis. A close relative may register the child if a parent cannot attend. " Starting at 6 p.m., in the order in which you arrived, you will sign-up and receive a number. This number will be attached to your application form and will verify the order in which your application will be processed. " Once you receive the number verifying the order in which your application will be processed, you can register anytime after 6 p.m. " Since not everyone can be registered at once, you are encouraged to take a tour of the school, watch a power point presentation about our schools, visit our information table, sign-up for our $50 tuition gift voucher door prize, meet our principal, teachers, staff and student ambassadors, and learn about the preschool program. We want you to feel welcome and learn about our school. Class Size " Three-year-old classes will be limited to 18 students and four-year-old classes to 20 students. Priority is given to families of the local Catholic parishes and those with siblings already enrolled in SMCESS. Specific teachers may not be requested. Kindergarten Registration - You Must Bring 1. A state verified birth certificate. Children must be 5 years of age on or before August 15, 2009 in order to register. 2. The registration form completed with all information requested. Please print legibly and sign the back of the form. 3. Immunization records of the child - see below. A nurse will verify that the record is up-to-date. 4. A $10.00 non-refundable registration fee. 5. The child's baptismal certificate (if baptized outside of local Catholic parishes) Registration Notes Full and shortened day classes will be offered with approximately 20 students per class. Priority will be given to families of local Catholic parishes and those with siblings already enrolled in SMCESS. A specific teacher may not be requested. Pennsylvania School Immunization Requirements: Children ENTERING SCHOOL (at K or grade 1) need the following vaccines: 4 doses of tetanus* (1 dose on or after the 4th birthday) 4 doses of diphtheria* (1 dose on or after the 4th birthday) 3 doses of polio 2 doses of measles** 1 dose of mumps** 1 dose of rubella (German measles)** 3 doses of hepatitis B 1 dose of varicella (chickenpox) vaccine or history of disease (with the first dose given on or after the first birthday) " Usually given as DTP or DTaP or DT or T " ** Usually given as MMR with the first dose given on or after the first birthday Please call the school office, 834-4169, if you have any questions. |
Transfer students are most welcome to join us, but parents and the student must know that the transfer from one school to another is a very serious educational decision. In all cases, the primary reason for a transfer (other than a family move) should revolve around the educational and/or spiritual benefits that a child would receive. Any student who is requesting transfer will be evaluated for admission based on full disclosure of his or her academic, attendance, behavioral, and disciplinary records. A meeting with the school principal will offer parents and the child the opportunity to ask many questions and discover much about the school. Tours are given at the convenience of the family. Also, the child is welcome to spend a school day shadowing a student at the appropriate grade level. Then, if a transfer is desired, three things must be done through the school office. " A Registration Form must be completed with all information requested. " A "Memorandum of Understanding" concerning the Catholicity of our schools must be signed. " A Request for School Records must be filled out and signed. |
